Application for Permit for Supervised Public Display of Fireworks


    This application will be approved only after all applicable forms are received and the proposal site has been inspected and all are in compliance with the Fireworks Regulation Section 6.4-37 of the Municipal Code of the Town of Normal, Rules of the State Fire Marshal, and all Illinois State Laws.

    Submit application to Normal Fire Department Prevention at least three (3) weeks prior to the date of occasion.

  2. Please provide the date and time of the pyrotechnic event.
  3. Please provide the name and address of the person or organization requesting the pyrotechnic display.
  4. Please provide the location name and address where the display will be taking place.
  5. Please provide the name and address of the licensed pyrotechnic firm.
  6. Attach a diagram of display site, copy of insurance papers and amount for bond or bonding company papers, and a copy of the pyrotechnic state license. If not submitted digitally, these must be submitted by mail or in person to the address above for approval.
  7. Has liability insurance been obtained?*

  9. All accidents must be reported to the office of the State Fire Marshal withing Thirty-six (36) hours of any occurrence.
